Diagrama Fostering is urgently seeking new foster carers. There are around 70,000 children in care in the UK at the moment and we need to find new carers to help meet current demand for placements. Children who need to be fostered come from a range of different backgrounds, cultures and ethnicities. Sometimes they are babies or infants, often they are older children. Some will be on their own, but others will be part of a family group. Some will have special needs or disabilities; others will have less obvious needs.

At Diagrama Fostering we support and empower people to provide homes which enable children to feel safe, secure, attached and inspired – where they’re safe and can start to enjoy life again.

Independent Social Workers – adoption assessments

Are you a qualified children and families social worker with experience in adoption assessment and support? We’re looking for Independent Social Workers to assess and prepare prospective adopters in and around the Kent area. We would particularly like to hear from you if you have experience of writing robust assessment reports and have a good understanding of therapeutic parenting approaches.

If you are a highly motivated and committed Social Work England registered social worker with the passion and dedication to help us find the best adopters for children, we would love to hear from you. We are seeking applicants who are able to drive or travel around Kent and the surrounding areas.

Diagrama Foundation is seeking an Instructor for our centres offering day opportunities for people with a learning disability, physical and sensory impairment, acquired brain injury and/or autism. This is an exciting opportunity to provide safe and professional person-centred care and support to service users to ensure they are supported and guided to develop independent skills, especially in the field of woodwork and land management; promote positive relationships; and aid their emotional, social, behavioral, physical and intellectual growth. 

The role will also include delivering activities around basic life skills, livestock care and gardening so experience in these areas would be beneficial, or the willingness to learn.

Key Responsibilities/Accountabilities

• To provide a safe and secure environment for the care and support of service users, ensuring compliance with all policies and procedures in place to meet our statutory and contractual requirements

• Plan, Create and produce activities to occupy and develop service users to ensure that they gain skills and education in line with their identified needs.

• Conduct service users strength-based assessments and ensure regular review of care and support plans.

• Complete, maintain and supply documentation to ensure relevant information is accurately recorded in line with company procedures, and compliance with statutory and contractual requirements.

• Build and create positive relationships based on mutual trust and respect with service users to ensure that they develop and grow in line with their identified needs.

• Communicate and act to promote teamwork and ensure high standards of service delivery in accordance with the best interests of the service user.

• Ensure own and, when appropriate, other’s actions reduce risks to health and safety in compliance with statutory legislation and company policy.

• Define structures and boundaries for service users to ensure their welfare; ensuring they understand behaviour expected.

• To assist in creating opportunities, within and outside the service for the service users to learn and develop from.

Diagrama Foundation is seeking a Development Worker for Duckyls Residential Care home, our site based on a farm near East Grinstead. 

This is an exciting opportunity to provide safe, professional and high quality person-centred active support to clients, working with them to ensure they are empowered to take control of their life, promoting autonomy, independence, and development of new skills.

This role will support access to a wider range of opportunities and engage in activities alongside others.

Key Responsibilities/Accountabilities

• Provide a safe environment in which to support clients, ensuring compliance with policies and procedures to meet our statutory and contractual requirements.

• Create and deliver activities that support clients to develop their skills and capabilities, adapting these and the environment to the individual needs of clients, to enrich their lives and promote choice within the home and in the community.

• Build and maintain positive relationships with clients based on mutual trust and respect to ensure emotional and social support is provided to the client to develop positive behaviours.

• Promote an environment which encourages positive relations with clients’ family and friends to ensure a diversity of support and integration for the client.

• Share the daily life of clients, providing personalised support to them in all living activities (including personal hygiene, cooking, eating, care of the home, laundry, shopping, managing finances, accessing professional services such as GPs, dentists), to ensure each client’s individual needs are met so they can live as normal and healthy a life as possible, and promoting choice and autonomy.

• Complete, maintain and supply documentation to ensure relevant information is accurately recorded in line with company procedures and compliance with statutory and contractual requirements.

• Contribute to the initial and ongoing assessment of clients’ needs and capabilities, through careful observation and sharing of information across the team to ensure that all aspects of support for clients continues to be tailored to their individual needs and goals.

• Communicate and act to promote teamwork across staff and with other professionals, ensuring high standards of service delivery in accordance with the best interests of the client.
