Championing Vulnerable Lives

Using our voices to shape the future of social support

Our Services

Thought leadership isn’t just about having ideas – it's about real impact

Through insight, experience and evidence-based approaches, we’re reforming social care.

Putting Ideas into Practice

  • Shared Learning
We bring together learning from our care homes, fostering, adoption, and young people’s services to set new standards and share proven methodologies that improve the lives of those we support.

  • Collaboration
We actively partner with experts and communities, such as frontline professionals, to co-create forward-thinking strategies that meet real-world needs.

  • Leading Conversations
We are an active voice, helping to shape the social conversation and amplify the voices of the vulnerable in a bid for meaningful reform.

  • Nurturing Future Leaders
We foster a culture of learning and ethical leadership among our teams, equipping them to challenge the status quo and influence the future of care.

How We Achieve Thought Leadership

Diagrama Foundation leads the way with youth rehabilitation

Lord Bird visits Fundacion Diagrama to see how our successful model leads to an 84% recidivism rate.
Diagrama Foundation, part of Fundación Diagrama in Spain, has spent over 30 years developing a different approach centred on rehabilitation, education, and reintegration.

“Our experience running secure re-education centres across Europe has shown that change is possible when young people are treated as individuals with potential.

We believe the system can - and must - do better. Together, with the government, partners, and sector leaders, we can create environments that are truly safe, supportive, and focused on long-term outcomes. A system that restores hope and changes lives.”
David McGuire, CEO.

Monument

‘A Chance for Change’ is the 5th book in the Monument Fellowship Series, sponsored by the Monument Fellowship whose 8 members are funded by the Woolbeding Charity to work together to make a sustained, cumulative and transformative change to the journey of individuals through our justice system.

‘A Chance for Change’ explores the important topic of what a fairer justice system could mean for children and young people through a collection of thought-provoking essays and creative reflections.

Published by Diagrama Foundation in conjunction with the Monument Fellowship, this book opens important dialogue about much needed changes to youth justice in the UK. It is a collaboration of work with a foreword from John Bird, Baron Bird MBE, founder of The Big Issue, member of the House of Lords and Diagrama Patron.

Diagrama Foundation offers a successful blueprint for alternative juvenile detention, which has flipped youth recidivism rates in Spain. We have published this book to explain the impact that the work of Diagrama and other organisations has on children and young people and inform them about emerging practices and new ideas.
