Diagrama Foundation runs 8 Supported Living schemes for adults with a learning disability across Bromley and in West Sussex. Providing a person-centric tailored support to individuals, we focus on enabling independence, choice, skills and enjoyment so everyone can live their best life and thrive.

Supported Living

Our Supported Living Services in Bromley, Orpington and East Grinstead focus on supporting vulnerable people to develop the skills and confidence to live in their own house either on their own or with others so they can lead independent, fulfilled lives.

Because everyone is different, our person-centred support is designed around each individual’s unique needs, enabling them to live the life they choose.

We provide a safe and stimulating environment in high-quality buildings, with spaces that support nurture and allow privacy as well as common areas and spaces to be active.

Diagrama Foundation is regularly inspected by the Care Quality Commission (CQC).

Diagrama Foundation provides expert quality care

We work closely with partners and local businesses to create meaningful work and volunteering opportunities. We work closely with each individual to explore the new day service options in the borough and developed tailored opportunities both within the home and in the community, including volunteering, work placements, daily living skills sessions, sensory activities, and exploring the local area.

Diagrama Foundation partners with Dartford Football Club

Staff support individuals to continually seek enjoyable and engaging community activities, including attending other day services; trips to local and London-based events, free community activities, sports events and collaborations with other organisations.

Community engagement includes involvement with Bromley Football Club, race track days, local theatre and shows, participating in exhibitions and projects across Europe including arts festivals in France. Our marketing team supports these initiatives by networking locally and promoting opportunities for individuals to explore.
